Jay Slotkin. MD, MPH, CMD is a board certified internist, geriatrician and certified medical director with over 40 years of clinical experience practicing as a medical director and attending physician in nursing homes and rehabilitation centers. He received his medical degree from SUNY Upstate Medical Center in Syracuse, NY and completed his residency training at George Washington University in Washington, DC. Following his residency training, he completed an MPH in Health Administration from the University of North Carolina at Chapel Hill before settling down in private practice on Long Island, NY. He has worked in many leadership roles including as President of the Medical Staff at Eastern Long Island Hospital. He has been a long-standing member of the Medical Society of the State of New York (MSSNY) and has served as the Chair of the LTC Subcommittee since 2018. He is past president of the New York State Medical Directors Association (NYMDA) and serves as an active member of the policy committee. He is presently consulting with an ACO on how to collaborate with nursing homes and remains actively involved in education with medical students, residents, physician assistants and nurse practitioners as Clinical Associate Professor in Medicine at SUNY, Stony Brook, NY.

Jane Winston, MD, AAFP, CMD is a native of Wahpeton ND, and a graduate of the University of North Dakota (UND) School of Medicine and Health Sciences.  She completed an internship at Hennepin County Medical Center in Minneapolis MN, a family medicine residency program at the UND Family Practice Center in Fargo ND, and a geriatric fellowship at Sun Health/St. Joseph’s in Phoenix AZ.
Dr. Winston is Board Certified in Family Medicine and holds a Certificate of Added Qualifications in Geriatric Medicine.  She is a Certified Medical Director, and a Clinical Assistant Professor in Geriatrics and Family and Community Medicine at UND School of Medicine and Health Sciences. Dr. Winston retired from her geriatric medicine practice at Sanford Health in Fargo in 2020 just before the Covid-19 pandemic began. 
As an advocate for age-friendly care for older adults, she volunteered and served on the North Dakota Medical Association Covid-19 Physician Advisory Group and the American Medical Directors Association (AMDA) Covid-19 State Task Force. In 2021 she was appointed to the AMDA State-Based Policy & Advocacy Subcommittee.  She is also a member of the Maricopa County Department of Public Health Medical Reserve Corp and serves as a volunteer vaccinator. She and her husband reside in Fargo ND and enjoy time away in the winter in Arizona. She has 3 stepdaughters and 4 grandchildren and one great-grandchild.  Dr. Winston also enjoys reading, cooking, walking, yoga, and traveling.

Daniel Haimowitz, MD, FACP, CMD, is a board-certified internist and geriatrician and has been in private practice in Levittown, PA, since 1986. Dr. Haimowitz is the medical director of several area nursing homes and assisted-living communities. He founded the Bucks County Long-Term Care Consortium and serves on the Bucks County Senior Task Force. He is also the medical adviser for the Bucks County Area Agency on Aging. A nationally recognized expert in long-term care and devoted advocate for older adults, he has participated in past White House Conferences on Aging, most recently in 2015. Dr. Haimowitz received the American Geriatrics Society Physician of the Year Award in 2001 and the William Dodd Founder’s Award for Distinguished Service from the American Medical Directors Association in 2009. He has served AMDA in many key roles over the years including as a member of the Board and on the public policy committee. He continuously assists AMDA in key initiatives and is very active in advocacy and public policy at the state and national levels.
